Objective: Since April 2021, there has been significant increase in number of COVID-19 cases in India. As the caseload increased, so did the complications like pneumomediastinum (PM) and subcutaneous emphysema (SE).Case Presentation: We present clinical course of 4 patients as case series of COVID-19 pneumonia who developed PM and SE during their management. Two cases with the need of non-invasive ventilation (NIV) recovered and two cases that needed invasive mechanical ventilation (IMV) ultimately expired.Conclusion: This case series highlights the importance of positive pressure ventilation via NIV and IMV as a predisposing factor for PM and SE as well as the need of strict vigilance by clinicians managing COVID-19 patients in IMV.

Objective: One of the most frequent causes of death and acquired disability in the pediatric population is the Traumatic Brain Injury (TBI). TBI is secondary to falls, road traffic and vehicle collisions, child abuse and assaults. Penetrating brain injury is a severe form of traumatic brain injury. Blunt head injury is more frequent than TBI in children, but the second one carries a poor prognosis and a worse outcomes.Case Presentation: We report a pediatric case of penetrating craniofacial trauma, caused by a pencil in to the eye, and the successful of multidisciplinary management.Conclusion: Traumatic head injuries in children are frequently seen in the emergency ward, but the penetrating head injuries are unusual in young children. This case shows the importance of the ‘first golden hour’ after head trauma. The collaboration between team members, added to a promptly and invasive strategy, allowed to reduce neurological sequelae.

Objective: Anterior teeth are of utmost importance with regard to chewing, speaking, and beauty. Therefore, any traumatic dental injury has psychologically undesirable effects on children and their parents. The present study reported a case of a traumatic dental injury induced by falling off a bike and referral to an emergency department as well as measures taken for dental reconstruction and patient’s improvement.Case Presentation: An 8-year-old child was taken to an emergency department due to falling off a bike following imbalance, while suffering from dental pain in his upper jaw due to dental subluxation. Afterwards, dental retaining paste along with a fixer wire was used to maintain the position of the teeth. The patient also received oral non-steroidal analgesia (acetaminophen) for proper control of the pain and was subsequently referred to a dentist to repair the teeth. The initial examinations revealed that the child was fully conscious. Considering the patient’s severe pain and intrusion of the superior left primary first tooth and the subluxation of the superior right primary first tooth. Local anesthesia was performed using 1% lidocaine in the location of the given teeth using infiltration technique. Then they were brought into an alignment by extracting the plunged tooth and the child was referred to a dentist for advanced dental trauma care.Conclusion: Dentists are not always present in hospital emergency departments; therefore, an emergency medicine specialist should be able to diagnose common traumatic dental injuries in order to manage them appropriately at the early stages, and if necessary, refer patients to dentists for further complementary treatments. This important fact will bedone after full examination and ruling out critical causes of trauma in the emergency department.

Objective: To demonstrate the significance of fused cervical vertebrae in emergencymedicine practice.Case report: A 35-year-old male was brought to the emergency department (ED) whoattempted suicide by hanging and was later found to have congenital fusion of cervicalvertebra C2 and C3, a condition referred to as - vertebra critica. This is the only case reportof a patient with vertebra critica admitted for near hanging. The patient was intubatedwith a Portex® North Polar endotracheal tube (ETT) through the nasal route. This proved tobe a critical decision as it helped early airway control avoiding any complication.Conclusion: As the victims of near-hanging should have cervical spine restriction,judicious use of flexible portex® ETT may help in early intubation and resuscitationwithout aggravating the neurological injuries. It must be noted that nasal intubation iscontraindicated in base of skull fractures and it should be avoided in patients with obvioustraumatic injury to the neck including laryngeal trauma, tracheal disruption and vesselinjuries.

This communication describes two attacks by domestic and wild carnivores in Caceres County, localized in the Pantanal area, an extensive flooded plain in Mato Grosso State, Midwest region of Brazil. The first attack took place in an urban area and was caused by a Rottweiler dog (Canis lupus familiaris) created by the family of the victim. Another attack occurred in a rural area, caused by a jaguar (Panthera onca), which was disturbed while feeding, characterizing an unprovoked attack. Both the patients were wounded in the forearm, with different severity of the wounds. The lesions restricted to the forearm were the consequence of the victims using their arms as a shield, featuring the “defense” injuries, but the severity of the wounds were proportional to the strength of the attacking carnivore. Health staffs and emergency centers should be alert for soft tissue and bone injuries in the observed body regions after domestic and wild animal attacks.

Objective: objectives: Penetrating abdominal trauma secondary to bamboo stick injury is a rare entity in the developed world. In developing countries like India, where nearly 70% of the population reside in rural areas, it is not that uncommon. Aim of this report is to recognition of risk factors and explain optimum surgical management in trans-abdominal bamboo stick injury.Case Presentation: Here we describe a case of penetrating trans abdominal bamboo stick injury resulting due to fallover a branch of bamboo stick with multiorgan damage, which was successfully managedby early surgical exploration in our institute.Conclusion: There are many factorswhich can prognosticate the penetrating trauma cases. The most important factors includea long interval between injury and surgical exploration, presence or absence of shock, andother associated organ injuries at the time of admission. Early recognition of the severity ofinjury with surgical management is utmost necessary. These patients have high morbidityand mortality rates due to multiorgan damage and post-operative infection

Statins are commonly used drugs in the treatment of hyperlipidemia (HL), despite some undesirable side effects. These range from mild symptoms such as myopathy, muscle weakness and myalgia to severe muscle weakness associated with chronic myopathy and acute renal failure (ARF) as a result of rhabdomyolysis. The most serious and deadly side effect of statins is rhabdomyolysis. The case presented here is of a patient with rhabdomyolysis due to treatment with the antihyperlipidemic drug, atorvastatin.

The COVID-19 pandemic will have numerous consequences in the management of patientswith genitourinary cancers. As we are struggling to best exploit our limited health-careresources in managing COVID-19 infected patients, we need to rethink our approach toprovide the best medical care for cancer patients in the time of this global crisis. Thereis a need for a decision-making algorithm which takes into account the age, presentingsymptoms, comorbid illnesses, access to health-care services, and availability of qualifiedhealth-care personnel. We must provide support, a clear and comprehensible informationalong with essential care to the patients seeking medical opinion during a time of thisunforeseen global health crisis. In this mini review we have made an attempt to prioritizethe necessity of intervention in urological oncology patients as per various national andinternational guidelines. This approach should be tailored as per locally available healthcare resources and the burden of COVID-19 infected cases in that region.

Objective: Clostridium botulinum is one of the most common life-threatening agents all around the world which produces botulinum neurotoxin (BoNT). It may lead to morbidities such as paralysis and mortality after consuming canned tuna fish, local dairy products and home-preserved or commercial food. People use a good deal of local dairy products such as cheese and it exposes a variety of population to botulinum intoxication.Case Presentation: A 48-year-old woman referred to the ED complaining about acute dysphonia accompanied with symmetric hypotonia and bilateral ptosis. She had a history of consuming local dairy cheese.Conclusion: It is indispensable for the physician to identify major features of foodborne botulism in order to differentiate it from other illnesses and early initiation of intensive care for intoxicated patients. To prevent the incidence of dairy product derived botulinum, it is firmly suggested to have quality control on local dairy products

Objective: Since the identification and spread of the novel coronavirus disease 2019(COVID-19) in December 2019, respiratory presentations have been introduced as themain symptoms of this new type of viral disease; however, the extra-pulmonary featuresare raising awareness for researchers due to the vast diversity of vital organs affectedby the virus. Among the wide range of clinical manifestations, limited data are availableregarding rhabdomyolysis (RML) in COVID-19.Case Presentation: In this report, we present a 58-year-old woman with COVID-19presenting with RML, with extremely elevated creatinine phosphokinase (CPK) and lactatedehydrogenase (LDH) levels (3283 and 1280 U/L, respectively) as the second sign ofdisease. Since the onset of the COVID-19 pandemic, several COVID-19 induced RML caseshave been reported, and timely diagnosis and proper management are of paramountimportance.Conclusion: Due to the findings that rhabdomyolysis can be a critical and missed cause ofmyalgia in COVID-19 patients, the importance of checking the serum level of CPK in patientswith myalgia and fatigue in the era of COVID-19 upon their arrival will be highlighted.

Carotid artery thrombosis and concomitant brain infarction after blunt trauma are rare conditions. We report a 34-year-old woman with multiple traumas due to pedestrian car accident. At the time of initial visiting, she was somnolent and her Glasgow coma scale (GCS) score was 14. Initial vital signs (V/S) and brain computed tomography (CT) scan were normal. One day after admission to the emergency observation unit, GCS improved to 15 but right-sided hemiparesis occurred. Magnetic resonance imaging (MRI) showed brain infarction and Doppler sonography indicated internal carotid artery thrombosis. She was admitted in the Neurosurgery ward and underwent anticoagulant therapy. With appropriate treatment, the patient’s condition improved after 5 days of admission and she was discharged on oral warfarin treatment and close follow up. Although there is no ideal treatment for traumatic internal carotid artery thrombosis, it seems that in most cases anticoagulation therapy is the preferred method of treatment but in some patients surgical or endovascular revascularization is indicated.

Objective: Tramadol is a synthetic analgesic with two mechanisms. The opioid and non-opioid mechanisms are responsible for tramadol side effects. Non-opioid side effects of tramadol are due to the reuptake inhibitions of serotonin and norepinephrine. Some of the side effects include anaphylactoid reactions, CNS depression, hypoglycemia, hypotension, respiratory depression, seizures, and serotonin syndrome. Seizure may happen in therapeutic doses. If the frequency of tramadol seizures increases, ischemic brain injury and hypoxic-ischemic encephalopathy can be induced.Case Report: We report a young man with a history of tramadol abuse that was admitted with status epilepticus in Imam Reza hospital in Mashhad, Iran. Due to his altered mental status, he was intubated and antiepileptic agents were prescribed. He was transferred to ICU. After regaining consciousness, he was extubated and with the prescription of rehabilitation support he was discharged.Conclusion: Tramadol is a synthetic analgesic agent with less potential for dependence. It is important to mention that the overdose of this drug is common. This drug has two mechanisms. This paper reports a case that developed generalized tonic clonic seizures due to tramadol and hypoxic ischemic encephalopathy. With adequate treatment and supportive care, patient’s mental status improves and he/she can be discharged.

Objective: Numerous drugs and medications from various pharmacological classes can lead to seizures as an unwanted side effect. Midwakh is a pipe commonly used to smoke tobacco blend in small quantities. Midwakh use is increasing, especially among young people. Case Presentation: A 17-year-old patient with a history of seizures was provoked by smoking midwakh, despite having no previous disease history. Conclusion: Although the patient had a negative workup for epilepsy, midwakh should be classified as an epileptogenic drug. More studies need to be conducted on the effects of midwakh on the neurological system

Objective: N-acetylcysteine is frequently used as an efficacious antidote for acetaminophen toxicity. It prevents liver injury induced by paracetamol and in most cases the overdose of acetylcysteine produces mild clinical effects.Case Presentation: Here we describe a patient who self-treated himself by acetylcysteine after acetaminophen toxicity. Approximately 5 hours after receiving 140 g of acetaminophen, the patient developed confusion, hypotension as well as seizures and also had coagulopathy and acute kidney injury. Other causes of these symptoms were overdose of acetaminophen and amoxicillin. Finally, the patient was treated by extensive supportive therapy and got healed.Conclusion: This case suggests that massive IV acetylcysteine overdose can cause serious life-threatening conditions. The purpose of reporting this case is to increase the awareness among medical staff concerning adverse reactions revealed after a massive overdose of N-acetylcysteine and their arrangement as well as describing the way of management of such problems. The seizure was one of the manifestations in our case and it is so rare. This indicates that massive dosing of acetylcysteine could form irreversible damages in the brain, so it is very important to start the management as soon as possible and monitor patients precisely.

Objective: Since the outbreak of coronavirus disease 2019 (COVID-19), the triage ofpatients diagnosed with corona virus has been a very important issue. The aim of thisstudy was to introduce a triage scoring system according to the clinical and para-clinicalfindings of patients in order to be admitted or discharged with COVID-19.Methods: After confirming the positive polymerase chain reaction (PCR) test for patients,we used a scoring system which included: the age of patient (less than 40 years and >40years), early vital signs at the time of admission, lab tests including C-reactive protein (CRP),white blood count (WBC), lactate dehydrogenase (LDH), D-dimer, chest imaging findings,comorbidity and shortness of breath.Results: The clinical score obtained for each variable in this scoring system was a numberbetween 0 and 3. The total score was a minimum of 0 and a maximum of 17. A higher scoreindicated an increase in the intensity and the need for intensive care. These scores wereclassified into 3 groups: 0-4, 5-10 and above 10. In the next stage, patients were dividedinto three groups: mild, moderate and severe. In this regard, patients with mild symptomswere suggested to receive home quarantine and home treatment, patients with moderatesymptoms were recommended hospitalization and medical care, and finally patients withsevere symptoms were inclined to intensive care.Conclusion: In order to treat and manage patients with COVID-19, it is necessary to payparticular attention to clinical and para-clinical findings and prioritize these findings basedon the severity and the condition of patients.

Arterial ischemic stroke (AIS) in adults is considered a serious health threat and requires urgent medical treatment. Prompt diagnosis allows the therapeutic option of thrombolysis within the time window of 3 to 6 hours after first symptoms. Alternatively, early anti-platelet therapy is effective in improving the outcome after stroke. The incidence of pediatric AIS range from 2 to 5 per 100 000 children/year. Pediatric AIS has severity and long-term outcomes similar to those in young adults. Two-thirds of children sustaining AIS have neurological deficits that may result in life-long disability, thus critically impacting their potential development. On the other hand 10%-30% of the causes of acute strokes are of unknown reasons, therefore careful structural, metabolic and genetic risk factors, requiring more specific treatment, should also be considered in any cases of stroke in children. The diagnosis and treatment should be conducted on the basis of a multidisciplinary approach, including pediatric cardiology, hematology, neurology, neurosurgery and neuroradiology.

Objective: During the late 2019, a group of patients had unexplained chest infections in Wuhan which turned out to be the new pandemic coronavirus disease 2019 (COVID-19). New neurological symptoms have been reported in COVID-19 patients. Posterior reversible encephalopathy syndrome (PRES) is a new neurological finding and is associated or caused by COVID-19.Case Presentation: A 32-year-old lady, with no medical background had COVID-19 infection and needed mechanical ventilation. After surviving the intensive care, she started to have multiple seizures that required general anesthesia to be aborted. The patient turned out to have PRES.Conclusion: PRES is a neurological syndrome causing seizures, headaches, and blurred vision. It is usually associated with high blood pressure, renal failure, and other risk factors. The patient in this case had nearly normal blood pressure, but still had a diagnosis of PRES. The new reported neurological symptoms associated with COVID-19 infection need further research and attention from the academic society to predict and prevent the morbidity and mortality of COVID-19 patients.

Objective: Renal infarction which is a rare phenomenon can be missed on the patient’s work up making its incidence low.Case Presentation: We report a 68-year-old lady with known history of hypertension presented with epigastric pain and vomiting for 4 days. During her stay in the emergency room, she developed sudden onset of left-sided weakness. Electrocardiogram was normal sinus rhythm. Computed tomography scan of the abdomen showed acute pancreatitis with partial impingement of superior mesenteric vein.Conclusion: Imaging s showed cholelithiasis without evidence of cholecystitis and lower pole right renal infarct likely secondary to obstruction of the right lower renal artery due to calcified plaques at their origin from the aorta. Subsequently, the patient was kept on antiplatelets and planned for discharge uneventfully.

Objective: Spontaneous uterine artery rupture in a non-pregnant woman is an extremelyuncommon event. To date, there have only been a few cases. Patients often present withacute abdomen and are hemodynamically unstable.Case Presentation: A 42-year-old female presented with multiple episodes of syncopalattack associated with vomiting and generalized abdominal pain. Upon arrival, she washypotensive which required aggressive fluid resuscitation with crystalloids and bloodproducts. Urine pregnancy test was negative. In addition, ultrasound scan showedfree fluid in the abdomen mainly at the splenorenal area and organized clots aroundthe uterus. Computed tomography of the Abdomen revealed a moderate amount ofhemoperitoneum with hypodense clots at the left para-colic gutter and pelvis. Sheunderwent an exploratory laparotomy and intraoperatively noted bleeding from leftuterine artery with 1.2 L of hemoperitoneum with no other abnormalities detected.Intraoperatively, there was an estimated 5 L blood loss which required packed cell anddisseminated intravascular coagulation transfusion. Subsequently, the patient was sent tointensive care unit where she recovered well and was discharged home 5 days later.Conclusion: Spontaneous uterine artery rupture is an extremely rare occurrence with highmortality if there is failure to detect and intervene early

Objective: Methanol poisoning is a dangerous life-threatening event, manifested withvarious symptoms, sometimes very rare ones, that all should be addressed to preventmisdiagnosis of the methanol-poisoned patients.Case Presentation: A 21-year-old young man was brought to the emergency department(ED) with a generalized tonic-clonic (GTC) seizure and he was diagnosed with methanolintoxication. A non-contrast computed tomography (NCCT) of the brain demonstratedfindings similar to subarachnoid hemorrhage (SAH). After the brain CT, he had a GlasgowComa Score (GCS) of 3 and all brainstem reflexes were absent. Neurology consultantagreed with the diagnosis of pseudo-SAH. Brain death was confirmed by a positive apneatest within 24 hours of presentation.Conclusion: It is suggested that compression of dural sinuses due to severe brain edema,reduces the venous drainage and leads to venous engorgement, which appears highattenuated in the background of low-density edematous brain matter.

Introduction: Unilateral and bilateral tongue cyanosis usually occurs due to the Raynaud syndrome and in the underlying severe types of vasculitis and rheumatology.Case Presentation: The present study was conducted on a 54-year-old woman who referred to the emergency department with complaints of sudden and painless discoloration of the left half of the tongue. The patient had no history of disease other than diabetes controlled with glibenclamide. Clinical examination of the head and neck revealed evidence of unilateral cyanosis in the left half of the tongue without pain, whose discoloration did not improve with warming of the tongue.Conclusion: Cyanosis in the emergency department can be managed appropriately by considering some parameters including history taking, history of cyanosis occurrence, history of cardiopulmonary disease, cold sensitivity and history of rheumatologic diseases, presence or absence of nail clubbing, arterial blood oxygen saturation and arterial blood gas test results. These parameters can be effective in designing a treatment regimen, while differentiating the causes of central from peripheral cyanosis.

Objective: Rhabdomyolysis is an important etiology for developing acute kidney injury(AKI). Among the many varying reasons for rhabdomyolysis, electrical injury seems to bea lesser-known factor. The clinical presentation of rhabdomyolysis is usually in the formof severe and widespread pain, tenderness, weakness in the muscles and dark urine. Itis characterized by the disruption of cell integrity in myocytes as a result of widespreaddamage to skeletal muscles and the passage of intracellular components into thecirculation.Case Presentation: Here we presented a case report of a young man who hadrhabdomyolysis induced by electrical injury which is relatively less common among theother etiological factors with preserved renal functions. He had electrical injury relatedwounds on extremities. Urgent intravenous fluid therapy was initiated as soon as hisadmission to the emergency department (ED), without delay.Conclusion: AKI is very common due to the nephrotoxic effect of myoglobinuria and theprerenal status. It is rare that AKI does not develop in patients with a severe increase increatinine kinase. It is a very important point to start effective fluid therapy in a short time.

Heat stroke is the most complicated and dangerous amongst heat injuries that can lead to irreversible injury and even death with itself or with creating predisposibility to different diseases. The following case report depicts a patient who presented primarily with impairment of consciousness after walking 45 km in the summer heat to cross the Syria-Turkey border and later syncope. This case report aims to highlight the possibility of higher co-incidence with heat stroke and stroke.
